1. Complete Online Intake

- Click "I am not the patient"
- Select "I am the patient's legal guardian"
- Enter your information and your child's information.
- Include insurance information
- Include the first and last name of your child's Primary Care Physician (PCP).
2. Activate Patient Portal
After completing Step 1, you’ll receive an email to activate your child’s patient portal. Please complete this as soon as possible by creating a password and entering your child’s date of birth.
Then, go to the "Uploads" section of your patient portal and submit the following documents, which are needed to get started.

You’ll receive your new patient paperwork through the patient portal. Please complete it as soon as possible. The more detail you provide, the better prepared we’ll be to discuss your child’s history and concerns during the initial assessment.

After you have completed steps 1-3, we will reach out to schedule your child's initial assessment, which will determine your child's therapy plan and group placement. Please plan to be in the clinic for 90-120 minutes for this appointment.

At the end of the assessment, we’ll review the scheduling options available. Your child’s care plan will be tailored to their individual needs as well as your family’s availability. If our current schedule does not align with your needs, we will add your child to our waiting list, and our program coordinator will reach out as soon as a spot becomes available.

We strongly recommend contacting your insurance to verify your child’s autism-specific therapy benefits. As a courtesy, we provide a step-by-step verification form in your patient portal to help you understand your coverage and avoid any unexpected billing. Please note that families are responsible for any costs not covered by insurance.
