Payment is expected at the time of service. We accept payments via the patient portal or cash, check, or charge in person.
Equipment fee: Some evaluations and services require the use of materials that are not re-usable. These are not billed to insurance and is an additional cost. This can range from $15-$100 per evaluation. Items used in other services along with their costs will be discussed with you prior to their use.
Evaluations: Upon request, we will provide evaluation reports and detailed receipts including diagnosis codes and CPT codes. Evaluation reports take up to three weeks to complete.
For Families Seeking Therapy Services
Please download the Insurance Verification Form for the therapy service(s) you are seeking, then call your insurance company for the information required to complete the form. Once completed, upload the form to your patient portal or fax to 734-527-5981.
