Payment is expected at the time of service. We accept payments via the patient portal or cash, check, or charge in person. 

Equipment fee: Some evaluations and services require the use of materials that are not re-usable. These are not billed to insurance and is an additional cost. This can range from $15-$100 per evaluation. Items used in other services along with their costs will be discussed with you prior to their use.

Evaluations: Upon request, we will provide evaluation reports and detailed receipts including diagnosis codes and CPT codes. Evaluation reports take up to three weeks to complete.
